Program Consent and Release Form
This form is required for all participants, adult chaperons, young team members, and adult team members taking part in any Salesian Gospel Roads program. It provides the Salesians with Medical and Emergency Contact information in the event of any emergencies that may arise during the program. It also contains a Photo/Video release, allowing the Salesians to use photographs and videos of participants in future publications, on our websites, and in presentations.
